An example of open reduction and internal fixation of the first metatarsal shaft fracture
1. A longitudinal incision was made along the dorsomedial fracture end of the first metatarsal bone, about 4cm in length.
2. The exposure of the skin, subcutaneous tissue, and deep fascia were cut in turn, the long and short extensor tendons were extracted, the periosteum was stripped, and the fracture ends were exposed.
3. The two broken ends were fixed with reduction forceps, towel forceps or skin forceps, and reduction was achieved by longitudinal traction according to the direction of fracture violence and the principle of reverse violence direction.
4. In the dorsal and medial side of the metatarsal bone, according to the shape of the shaft and the direction of the fracture line, choose the appropriate anatomic Titanium Plate (the length of the two broken ends should leave 2-3 screw holes), fit well, press the axial force of the broken end, and implant the screws in turn to test the fixation is stable and effective, and there is no loosening of the broken end. For comminuted fractures, a mini External Fixator can be used.
5. Normal saline was used to flush the surgical orifice, electrocoagulation hemostasis was used, layered suturing was performed, the incision was closed, and sterile dressing was pressurized.
